It sounds like you are dong all the right things. Keep in mind that during pregnancy, your blood volume increases 40-60% which causes the blood vessels to dilate, so swelling is a normal part of pregnancy.
One of the reasons for swelling is prolonged stillness. Since you are experiencing the most swelling when you wake up, I would recommend gentle movements upon awakening. Try doing some wrist and ankle circles first thing in the morning.
Also, if you find that the swelling is lingering though out your day, continue to drink plenty of water, don't eliminate salt all together instead salt your food to taste, take brisk walks and try wearing support hose. You can also try cold compresses for your hands and feet or submerge them in a bath or pool.
