Arnica for Tears to Perineum
Most use the pellets rather than the cream for this type of wound. The cream is not supposed to be used on broken skin. I have known pp women and mothers to do it though and found it to still work without problems.
With the pellets, the mother can put 2-3 in her peri bottle before she fills it with water and use that solution when she pees. She can also take it orally (2 pellets) whenever she remembers to. Try to avoid taking around meals and touching the pellets with her hands. As she starts to feel better she will naturally start taking it less often because she is feeling better. Any dosage she can get will be helpful, but I think the stronger the better for this.
